Most of the time it's the software themselves who crash. And this can be because of installation problem, old version that is still buggy or things like that.
The icons bug was really strange indeed and in fact I just saw it one time at the WOASE. This can happen if someone had crashed a software before but the OS wasn't completly stalled but not working anyway.
In fact I can remember this happened also on AmigaOS 3.x sometimes when an app crash.
